Welcome to Midwest Community Care Inc.

The Organisation

  • Midwest Community Care is a not for profit organisation that has been providing community care services for frail older people, people with disabilities and their carers since 1989.
  • Midwest Community Care is governed by a community based Management Committee drawn from the local community and elected annually by the general membership of Midwest Community Care.
Midwest Community Care Inc currently auspices the following programs:
  • Midwest Community Options (including Dementia Options) – the role of this program is to provide case management/care coordination, monitoring and brokerage services to people with complexcare needs, including people with dementia. This program is funded by the NSW Department of Ageing, Disability and Homecare under the Home and Community Care (HACC) Program.

  • Midwest Compacks Program - Compacks is funded by NSW Health and provides a case managed package of care provided for up to 6 weeks after discharge from hospital. The service has been designed for people who need two or more community services to ensure that they can return home safely with appropriate community supports in place. The concept is to case manage community support jointly with a multidisciplinary team, starting before discharge and continuing for a short time after discharge, with the aim of facilitating access to mainstream community services.

  • Midwest Dementia Services Program – the role of this program is to provide support and assistance to people living with dementia in their own home. Fully trained Community Support Workers are able to offer support by providing a service to meet the needs of the individual with dementia. This program is funded by the NSW Department of Ageing, Disability and Homecare under the Home and Community Care (HACC) program.

  • Midwest Multicultural Dementia Carers Program – the role of this program is to provide respite for carers of people with dementia who come from an Arabic, Chinese or other multicultural background. Bilingual coordination staff and direct support staff are able to support carers by providing them with a break from their regular caring role. Services are tailored to meet the needs of the individual. This service is funded by the Australian Government Dept of Health and Ageing under the National Respite for Carers Program (NRCP).
