Community Options Program

What is the Community Options Program

The Community Options Program (COP) is funded by the Department of Ageing, Disability and Home Care under the Home and Community CareProgram (HACC). The program provides assistance to eligible people and their carers to be more independent within their home and community. COP is aimed at those people consideered as having complex care needs that are currently not met through existing services, and require a case management and co-ordination service.

Am I Eligible?

People who are able to access the services offered through the Community Options Program can usually answer yes to the following statements:
  • Do I have complex care needs including a combination of physical, emotional, social and medical needs?
  • Do I require assistance in organising and co-ordinating the services I currently receive?
  • Do my circumstances regularly change requiring my support services to adapt and change to my needs?
  • Do I find it difficult to access services due to my special needs?

Choosing the right option

Under the program we are able to provide the folling services and assistance:
  • Case management involving comprehensive assessment and service planning.
  • Service co-ordination acting as a central reference point and negotiating service levels and standards and monitoring provision.
  • Information and links to a wide range of available care and support services in your area.
  • Opportunity to purchase services on your behalf if a gap exists with current providers.
  • Flexible delivery responding to your changing needs and requirements.

Making the right choice

Our Community Options Program covers the areas of Parramatta, Baulkham Hills, Ryde/Hunters Hill and Holroyd. If you, your relative or friend requires a range of basic support services to maintain well-being and independance then contact on of our team and we will arrange the following:
  1. Take a referral.
  2. Arrange an assessment.
  3. Develop a care plan.

Are there any fees?

A fee based upon your ability to contribute to the costs of services offered may be negotiated. However if you are unable to contribute financially this will not preclude you from receiving assistance.
