Midwest Compacks Program

What is the Midwest Compacks Program?

The Compacks (short for Community Packages) program provides people with a case managed package of care for up to 6 weeks after discharge from hospital. The concept is to case manage community support jointly with a multidisciplinary team, starting before discharge and continuing for a short time after discharge, with the aim of facilitating access to mainstream community services.

Midwest Community Care can provide Compacks support for people returning to their home in any of the following Local Government Areas:
  • Parramatta
  • Ryde
  • Hunters Hill
  • Holroyd
  • Baulkham Hills

Who Is Eligible?

The service has been designed for people who need two or more community services to ensure that they can return home safely with appropriate community supports in place. The service is only available to patients in participating hospitals and referrals must be made by Social Work (or similar) staff from within the hospital.

What Services do We Provide?

The primary service provided is case management and the emphasis is on undertaking a comprehensive needs assessment and making linkages with a range of appropriate community services to assist the client to remain sustainable in their own home. Brokerage funds are also available to purchase a range of short term services to assist the client in their home immediately after they are discharged.
