Dementia Services Program

What is the Dementia Services Program?

The Dementia Services Program is designed to provide support and assistance to people living with dementia in their own home. Fully trained support staff are able to give carers a break from their dcaring role by providing a service designed to best suit the needs of the individual with dementia. Services may also be provided to people with dementia who live alone and require extra support to remain independant. Culturally speciifc services are available. The Dementia Services Program is funded through the Home and Community Care (HACC) Program and the National Respite for the Carer's Program.

Who is Eligible

People who are able to access the services offered through the Dementia Services Program can usually answer yes to the following statements:
  • Do I care for a relative of friend with dementia?
  • Do I know of someone living along with a dementing illness?
  • Do I sometimes need a break from my situation to catch up with friends and family?
  • Do I require further information and resources about Dementia to better understand my own situation?
  • Would I benefit from regular contact iwth a worker who understands my situation and is able to offer support and contact?

Choosing the right option

Under the program we are able to proivide the folllowing services and assistance:
  • Organising social and recreational activities.
  • Providing respite and time-out for carers.
  • Information and links to a wide range of available dementia care and support services in your area.
  • Regular contact and liason including monitoring and prompting
  • Assistance with daily living tasks including light house work, shopping and meal preparation.

Making the right choice

The Dementia Services Program offers a numver of choices for different members of the community. Culturally appropriate services (generally with a bi-lingual worker) are available for people from a Chinese, Arabic or other multicultural background. Services are offered for people living in the Local Government Areas of Parramatta, Holrody, Ryde and Hunters Hill.

If you, or your relative, friend or neighbour has a confirmed diagnosis of dementia and you would like support and assistance at home then please contact one of our team and we will arranage the following:
  1. Take a referral
  2. Arrange a home visit to assess your needs
  3. Develop a care plan to meet those needs

Are there any fees?

A fee based upon your ability to contribute to the costs of services offered may be negotiated. However if you are unable to contribute financially this will not prevent you from receiving assisstance.
